Where do I get a TB test?
As a condition of employment, you need to complete a TB screening within 30 days of employment. We utilize Honor Health/FastMed for TB screenings, and they bill us directly.
Download this HonorHealth TB Authorization & Map-PHX, fill in your name, and take it with you to Honor Health/FastMed location listed in the links below. If you cannot print it out, please contact Christine Dowd at HR@cfcare.org for assistance.
(If you are in Southern or Northern Arizona or in another region not close to an Honor Health/FastMed Clinic, you can complete a TB screening with your physician or at a wellness center (i.e. Walgreen’s Healthcare Clinic or the urgent care center of your choice). Save all documentation and receipts for reimbursement (this screening should be around $40-50).
