A COA fundamental practice of an organization that permits or requires the use of agency, or privately owned, vehicles to transport clients requires CFC to obtain annual validation of license, driving records and vehicle insurance. (ASE 4.02)
PLEASE FOLLOW ALL 3 STEPS BELOW:
- To Obtain your Motor Vehicle Record (cost will be $3.00/39 months) go to https://azmvdnow.gov/, create a login and find Drivers License Record under “Other Services” .
- Select the 39 Month option. Follow prompts. You may submit this cost for reimbursement. Please see supervisor for budget code.
- Save a copy of the report and upload it to your extendedReach profile.
- Upload a copy of a current vehicle registration into your extendedReach profile
- Upload a copy of a current Auto Insurance Declaration page into your extendedReach profile
If you do not have the ability to upload files to your eR profile, send these items to your Program Assistant. If you have any questions regarding these requirements, please see your supervisor.
